The Registrar’s Office assists families in enrolling at JCHS, processing transcripts and incoming credits from prior schools.
New Student Enrollment
All new students to USD 475 must complete the online registration. Please go to usd475.org. Find the enrollment tab and select New Student Enrollment or click the link New Student Online Enrollment
Once online registration is completed, the Registrar will receive notice of your enrollment request. Records will be requested from the student’s previous school. Once records from are received, the Registrar will contact you by phone or email to schedule an enrollment appointment. Please bring student’s birth certificate and immunization records to the appointment. Legal Guardianship papers are required if you are not the biological parent of the student.
Requesting Transcripts
Former students needing official or unofficial transcripts, should click on the link provided below. Please allow at least one week turnaround.
*Official transcripts can only be mailed or emailed to educational institutions, human resources personnel, or recruitment agencies.
*Please note that transcripts cannot be sent via personal email addresses; only hard copies can be sent for personal uses.
*Schools requesting records: Please email the records request to the Registrar.
Current students requesting transcripts can obtain a transcript request form at any academy office or see the Registrar in the Main Office.
